Modafinil: What to Know Before You Order Online

8 min read

Modafinil is a prescription drug in nearly every country. This guide is about safe use within whatever legal framework applies to you — not about evading laws.

Legal Status by Region

US: Schedule IV. Prescription required. UK: Prescription-only medicine. EU: Prescription required, varies by country. AU: Prescription required (Schedule 4).

The Contraceptive Issue

Modafinil induces CYP3A4. This reduces serum levels of estrogen-based contraceptives by enough to cause failure. The product label states this. Many users don't know.

Anyone on a hormonal contraceptive who starts modafinil needs a backup method (e.g., barrier) for the entire duration of use plus one cycle after.

Common Drug Interactions

- SSRIs/MAOIs: increased anxiety - Warfarin: modafinil may alter levels - Cyclosporine: reduced levels - Birth control: see above

Cardiovascular Screening

Modafinil should not be used by anyone with a history of left ventricular hypertrophy, mitral valve prolapse, or uncontrolled hypertension. A baseline BP check is reasonable before starting.

The Stevens-Johnson Risk

Rare (<1 in 1000) but severe. Any rash that appears in the first month is a "stop immediately and call your doctor" event.

Dose Strategy

100 mg AM first three days. If well-tolerated, 200 mg AM thereafter on use days. Avoid daily use long-term — 5 on / 2 off preserves response and reduces tolerance.
